COALITION TO PREVENT LEAD POISIONING
Founded in 2000 as the Lead Free Rochester Coalition, the
Coalition to Prevent Lead Poisoning (CPLP) is an education
and advocacy organization composed of nearly 100 individuals
and community organizations dedicated to eliminating childhood
lead poisoning in Monroe County. The coalition's mission is to provide
leadership and advocacy to empower the community and its residents
to prevent the lead poisoning of children by creating an environment
that is free of lead hazards, facilitates the creation of a system
that protects children, creates jobs, and enhances property values.

COMMUNITY HEALTH 2020 COMMISSION ON SYSTEM
PERFORMANCE
The Community Health 2020 Commission on System Performance
(2020 Performance Commission) will develop infrastructure to
support a patient-centered health care delivery system which
provides quality care for the entire community, reduce health
disparities, and manage demand for acute hospital services.
The 2020 Performance Commission will also establish improved
metrics to quantify and improve health status and system
effectiveness. The work of this Commission will develop out
of the recommendations of the 2020 Commission which was
convened in 2008, and focus on supporting approaches to reduce
the demand for inpatient hospital care in the region by
improving access to preventive and primary care services.

COMMUNITY TECHNOLOGY ASSESSMENT ADVISORY BOARD
The Community Technology Assessment Advisory Board (CTAAB) was
established in 1993 to augment the health care planning process
in the Rochester area and to continue the Rochester community's
legacy of high quality, affordable health care. CTAAB is an
independent board that reviews selected issues such as new or
expanded technology, new or expanded services, and capital
expenditures;makes judgments on these issues; communicates its
decisions to the health care community.

HEALTHI KIDS COALITION
Advocating for a handful of public-policy and practice changes,
the Healthi Kids Coalition blazes a path for healthier, more
active children in Rochester and Monroe County, N.Y. The
Coalition's change agenda calls for better school food, safer
play areas, food standards at childhood centers, at least 45
minutes of in-school physical activity, and policies that
support breastfeeding.

FLHSA/RBA HIGH BLOOD PRESSURE COLLABORATIVE
The High Blood Pressure Collaborative was created by the
Rochester Business Alliance Health Care Planning Team to
address these issues. In partnership with the Finger
Lakes Health Systems Agency, they are now being implemented
in the Finger Lakes Region.

THE PARTNERSHIP ON THE UNINSURED
The Partnership on the Uninsured was formed in the Spring
of 2000 with the goal of ensuring that all Monroe County
residents are continuously covered by an affordable health
insurance plan and able to access appropriate health care
services when needed. Currently the Partnership works to identify
barriers to insurance coverage, research and identify strategies to
reduce those barriers, take action/advocate for strategy
implementation, educate the community on health care access and
insurance coverage, and evaluate and report the effectiveness of

SAGE COMMISSION
The Sage Commission, in partnership with the work of the
Community Health System 2020 Commission has been convened
by the Finger Lakes Health Systems Agency to create a
strategic vision of person-centered aging services for the year 2020
and beyond. This vision will be detailed in a written plan that
addresses the health status and needs of the adult population age
65 and older who reside in the 9-county Finger Lakes region. Special
attention will be given to ways to minimize disparities in health
